New Hire Reporting Forms - Fill out these forms if you have been hired for the current summer. If you are a new employee, you must fill out an I-9 form which you can get at the South End Pavilion. Past employees will be notified when you need to re-submit an I-9.

Employment Application - Print and mail to address on the last page of the application. Please print and bring to Anthony

Federal I-9:

Must be completed in the office.
Need EITHER a birth certificate or social security card AND a school ID, report card, or drivers license to complete the form

Working Papers:

Can be obtained at your school. Follow directions given.
