To obtain a certified copy of a vital record, you must submit:
- A completed application
- A copy of the proof(s) of your identity
- The correct fee - $10.00 per certified copy requested
- Proof of your relationship to the person listed on the vital record you are requesting (Only required for certified copies)
To apply for a marriage or civil union:

Please print out and complete the license application form. You may also get an application form at the Local Registrar’s office. To be sure the Registrar is available, it is best to call ahead and schedule an appointment.
Do not sign the marriage application form before going to the Local Registrar. You must sign the application, while under oath, in the presence of the issuing authority.
You must also bring with you at the time of application a witness, over the age of eighteen (18) years who knows both parties.
There is a $28.00 application fee.
